This Privacy Notice applies to both GLBA and non-GLBA information, with specific distinctions noted in each section.

What Personal Information Do We Collect?

As used in this Privacy Notice, “personal information” means any information about an individual from which that person may be identified. Personal Information includes obvious things, like your name, telephone number, email address, and street address, as well as less obvious things like your IP address, device ID, and location information. Personal information does not, however, include information from which the identity of an individual has been definitively removed (also known as anonymous or anonymized data).

In order to provide the Platform and Services, we collect various types of information. We collect personal information directly from you when you provide it to us, such as when you contact us to inquire about or purchase insurance plans from our licensed insurance agents or our affiliate’s licensed insurance agents or you purchase related services with us or otherwise communicate with us verbally or in writing. We also may collect information indirectly, such as through automated technologies (e.g., cookies and similar tracking technologies). As well, we may receive your personal information from affiliate partners that you provided your information to directly.

Personal Information You Provide to Us

General Inquiries (Non-GLBA)

You can generally visit the Platform without having to submit any personal information about yourself to us. However, if you require more information about our Services, you may contact us via email, telephone, or otherwise, in which case we will collect your name and email address, as well as any other information that you submit to us. This information is considered non-GLBA information.

Insurance Plan Shopping, Comparison, and Purchase (GLBA)

If you wish to shop and compare insurance plans in your area through our website (PolicySlicer.com), we may request the following information:

  • Full name

  • Zip code

  • Date of Birth

  • Gender

  • Email address

  • Phone number

Should you choose to speak with one of our licensed insurance agents or our affiliate’s licensed insurance agents after your initial online inquiry, they may request additional information over the phone to assist you with finding suitable plans, which could include health-related information or your Medicare Beneficiary Identifier (MBI) number. This information provided over the phone is considered GLBA information and, where applicable, may also be subject to HIPAA.

This information may also be shared with a Healthy Labs Inc affiliate agency. This information is considered GLBA information.

Personal Information We Collect Indirectly

Automatically-Collected Information (Both GLBA & Non-GLBA)

When you visit our Sites, we (or our authorized service providers) automatically collect information about the computer or devices (including mobile devices) you use to access the Platform, and your interactions with the Platform. This includes your browser type, IP address, language, operating system, location of your wireless device (e.g., latitude and longitude), the state or country from which you accessed the Platform, unique device identifier (including click IDs), the pages you view, length of time spent on pages, the manner in which you use the Platform (e.g., the content you access, view, click on, search for etc.), the date and time of your visit, the websites you visited immediately before and after visiting the Platform, error logs, and other hardware and software information, as well as other geographic and demographic information. This is achieved through the use of cookies and similar tracking technologies.

We may use these tools to collect information, formulate statistical models about use of the Platform, enhance the Platform for our users, and provide you with information about new opportunities, and tailored content, advertising, and marketing.

For more information about our use of cookies and similar technologies, please see Cookies and Similar Technologies below. This applies to both GLBA and non-GLBA data collection.

Location Information (Both GLBA & Non-GLBA)

We or our authorized third–party service providers also collect broad geographic location (e.g., country, or city-level location) based upon IP address, in order to determine which licensed insurance products are available for purchase in your area, the popularity of products in different geographic locations, and to guide marketing strategies. This applies to both GLBA and non-GLBA data collection.

Please check your specific browser or device settings to disable location services if you do not wish to share precise location information.

Third Parties (Both GLBA & Non-GLBA)

We may collect personal information from third parties, such as our service providers, in connection with and in order to facilitate a transaction. This may be both GLBA and non-GLBA related.

Payment Service Providers (GLBA)

Healthy Labs Inc may collect payment for plans purchased on our Platform. We do not store your payment information. We use a third-party, PCI-compliant, payment processor, which collects payment information on our behalf in order to complete transactions. While we are able to view and track actual transactions, we receive a token from our payment processor and do not have access to or process your credit card information. This information is considered GLBA information.

Cookies and Similar Technologies

What are Cookies and Similar Tracking Technologies?

The Site uses “cookie” technology and similar online tools, such as web beacons and web pixels (collectively “cookies”). “Cookies” are small files that a website stores on a user’s computer or device and through which personal information may be collected. We use cookies to keep the information you enter on multiple pages together, to track user behavior, and to enable personalized advertising.

Some cookies are first-party cookies – meaning our cookies – and others are third-party cookies, meaning that they are placed on the Site by authorized third parties, such as advertising partners (e.g., Google). Most web browsers automatically accept cookies, but you may set your browser to block certain cookies (consult the instructions for your particular browser on how to do this). Please note that if you decide to block cookies, this may interfere with your ability to perform certain transactions, use certain functionality, and access certain content on the Site.

We use “session” cookies, which are automatically deleted from your hard drive after you close your browser. Session cookies optimize performance and limit redundant data downloads. We also use “persistent” cookies, which remain on your computer unless deleted. We use these for purposes such as statistical analysis, performance quality, and to enable targeted advertising.

Generally speaking, technologies that work like cookies include “pixels”, which many websites use to learn whether you’ve interacted with certain web or email content.

Some of the cookies or similar technologies that may be used are “essential” for the Site to function, while others are “functional” in that they allow sites to add additional functionality. Other cookies or similar technologies help us improve our Site and measure audiences through analytics, including via Google Analytics, as further explained below, or are used for advertising purposes. This applies to both GLBA and non-GLBA data collection.

Your Choices

Most web and mobile device browsers automatically accept cookies, but if you prefer, you can change your browser to prevent that or to notify you each time a cookie is set. Some browser manufacturers provide comprehensive help relating to cookie management in their products. Please see your web browser provider’s settings for more information.

You can also learn more about cookies by visiting www.allaboutcookies.org, which includes additional useful information on cookies and how to block cookies using different types of browsers or mobile devices.

Follow these steps to change your cookie settings in popular browsers:

  • In Chrome: Settings > Privacy and security > See all site data and permissions

  • In Firefox: Settings > Privacy & Security > Manage Data

  • In Safari: Settings > Privacy > Manage Website Data

Please note, however, that by blocking or deleting all cookies used on the Sites, you may not be able to take full advantage of the Sites and you may not be able to properly log on to the Sites. In addition, we cannot control how third parties track you across the Internet, irrespective of our activity. For California residents, please also see the "Right to Opt-Out of Sale / Sharing" section below regarding your choices for cookies used for targeted advertising.

Analytics

The Sites use Google Analytics, a service that drops cookies and/or similar technologies to collect and store information about your device and use of the Sites. We use Google Analytics to calculate visitor, session, and campaign data for analytics reports. You can read Google’s privacy policy [here](https://policies.google.com/privacy?hl=en-US#infocollect). You can opt-out from being tracked by Google Analytics on the Platform in a particular browser on a particular device by downloading and installing the Google Analytics Opt-out Browser Add-on for that browser, which is available [here](https://tools.google.com/dlpage/gaoptout). We do not anonymize IP addresses through Google Analytics by default, and data is stored on Google servers. This applies to both GLBA and non-GLBA data collection.

Social Media and Plug-ins

Our social media accounts may enable us to collect information from you when you interact with our accounts. We also have social plug-ins on our Sites and may include icons that allow you to interact with third-party social networks. The third-party social plugins generally set a cookie when your browser creates a connection to the servers of such social networks and may transmit your personal information to the social networks. Your use of these social plugins, and your use of the products offered by these social networks, is subject to the privacy policies of the third-party social networks. This applies to both GLBA and non-GLBA data collection.

How We Use Your Information

We use your information for the following business and commercial purposes:

To Provide Personalized Services To You (GLBA)

We use the GLBA information we collect to provide Services to you in a personalized way including offering a quote, processing transactions, maintaining and servicing your account or policy, and providing customer service. We may also use the information we collect to provide Services on behalf of financial service providers (e.g., insurance carriers, financial institutions). The information we collect gives us a more detailed picture of who you are so we can help you find the right product.

To Communicate With You (Both GLBA & Non-GLBA)

The information we collect may be used to communicate to you and with you for various purposes, which include but are not limited to:

  • Informational/Educational: Connect you with real people who can help walk you through your Service options, communicate to you about potential topics of interest, communicate to you about your financial service needs and interests.

  • Transactional: Facilitate and/or administer application, policy, or account information to you, inform you of product and Service information, and maintain a normal everyday business relationship.

  • Conversational: Chat with you for matters that you’ve requested, to provide a seamless customer service experience.

To Improve Our Services (Both GLBA & Non-GLBA)

We are constantly trying to improve our Services. The information we collect (including telemetry) may be used for investigations to address software defects, to conduct internal business research, and perform data analytics including data modeling. Specifically, we may use data and analyze user activities and behaviors to improve or adjust our Services, identify additional areas of interest, improve your experience, and improve the way with which we offer personalized recommendations.

To Detect Fraud (Both GLBA & Non-GLBA)

We respect your privacy and safety and therefore may use the information we collect to detect, investigate, prevent, or act on incidents, threats, and misuse of natural persons, your information, or our systems. This includes suspected theft, fraud, malicious, deceptive or illegal activities, or violations of our policies. As a result, we may also use the information we collect to verify your identity.

For Legal Reasons (Both GLBA & Non-GLBA)

We operate in a regulated industry and therefore reserve the right to access, use, preserve, transfer or disclose information related to your use of our Services as reasonably necessary to do any of the following: comply with applicable law; respond to complaints, legal actions or governmental requests; enforce our Terms of Service; protect our rights, privacy, safety, or property; or respond to other similar matters.

As Part of a Corporate Change (Both GLBA & Non-GLBA)

We may use information about you as part of, or in preparation for, a change of control, restructuring, corporate change, or sale or transfer of assets.

For Business and Commercial Purposes (Non-GLBA)

We may use non-GLBA information to:

  • To personalize your experience with our Sites.

  • To market or advertise new products and Services to you. This includes targeted advertising based on your browsing history, demographic information, and interactions on the Platform and via third-party advertising platforms. (See "Data Sharing" and "Additional Information for California Residents" for more details on sharing for advertising).

  • To create inferences and profiles to better understand your preferences.

  • To conduct internal research, reporting, and data analysis.

Data Sharing

We may share your data in the following ways:

With Insurance Carriers and Agencies (GLBA)

We may share your GLBA information with our business and financial service partners (e.g., insurance carriers) to provide and improve our personalized Services, including when you direct us to do so by applying for a product.

With Third Party Service Providers (Both GLBA & Non-GLBA)

We may share your information with third-party service providers acting on our behalf to help us operate our Platform and Services. This includes but is not limited to: insurance agencies, Agents and financial professionals (when facilitating services you requested), technology and infrastructure providers (e.g., website hosting, data analytics, communication tools), legal and compliance service providers, and process outsourcing companies. These service providers are contractually bound to protect your information and use it only for the services we have engaged them to perform.

With Marketing Partners (Non-GLBA)

We may work with third-party marketing partners to advertise our Services. In connection with these activities, when you interact with our Platform, information such as your browsing history, demographic information (excluding sensitive data like specific health information, MBI, or detailed financial status not essential for general marketing segmentation), and interaction events (like page views or clicks) may be shared with these partners, often through cookies, pixels, and similar technologies. This may include identifiers like click IDs that allow these platforms to recognize your device or browser for purposes of measuring ad effectiveness and delivering targeted advertising to you on other websites or platforms.

We do not share sensitive health information (as defined by us, e.g., specific conditions, medications), your MBI, or your full date of birth with these marketing partners for these specific marketing purposes. For California residents, this type of sharing may be considered "sharing" for cross-context behavioral advertising, and you have the right to opt-out (see "Additional Information for California Residents").

If you explicitly engage with an offer from a specific partner through our Platform (e.g., by clicking to request information from them), we will share the necessary information with that partner at your direction to facilitate that interaction.

For Legal Reasons (Both GLBA & Non-GLBA)

We reserve the right to transfer or disclose information about you to third parties for certain legal purposes as described above. We may also disclose and transfer information about you to a third party as part of, or in preparation for, a change of control, restructuring, corporate change, or sale or transfer of assets.

Do Not Track Signals

We do not currently respond to legacy “Do Not Track” (DNT) signals in browsers because a uniform technological standard has not been developed. However, for California residents, we process Global Privacy Control (GPC) signals as a valid request to opt-out of the “sharing” of personal information for cross-context behavioral advertising as described in the "Additional Information for California Residents" section.

Disclosure of Your Information

We may share your information in the following ways:

  • We may share information about you with the following types of companies:

  • Insurance carriers, agencies, and other financial services providers, to provide you with quotes, insurance plan information, and facilitate enrollment (GLBA).

  • Companies that perform services on our behalf, such as website hosting services, communication tools, or third-party content providers (Both GLBA and Non-GLBA).

  • Analytics partners, including behavior analytics services and advertising platforms (Both GLBA and Non-GLBA). In some cases, these third parties or service providers collect information directly from you via the Platform. (See "With Marketing Partners" above and "Additional Information for California Residents" regarding sharing for advertising).

  • We may share your information with any person or entity where we believe in good faith that such disclosure is necessary or otherwise appropriate to: (a) comply with the law or in response to a valid subpoena, court order, government request, or other valid legal process; (b) produce relevant documents or information in connection with litigation, arbitration, mediation, adjudication, government, or internal investigations, or other legal or administrative proceedings; (c) protect the interests, rights, safety, or property of Healthy Labs Inc or others; or (d) enforce our rights or other contractual obligations (Both GLBA and Non-GLBA).

  • We may transfer your personal information to a successor entity in preparation for or upon a merger, consolidation, or other corporate reorganization in which Healthy Labs Inc participates, or to a purchaser or acquirer of all or substantially all of Healthy Labs Inc’s business or assets, including a successor in bankruptcy (Both GLBA and Non-GLBA).

  • We may share your information with any person or entity when we have your consent, or when you direct us to share your information with a person or entity. This includes insurance brokers (GLBA and Non-GLBA).

  • We may use and share non-personal information about you or use of the Platform and Services, including any de-identified and aggregated data with third parties without limitation (Both GLBA and Non-GLBA).

We do not sell your personal information to third parties in exchange for monetary consideration. For information on "sharing" for advertising purposes under California law, please see "Additional Information for California Residents."

Our Role Regarding HIPAA

Healthy Labs Inc acts as a Business Associate under the Health Insurance Portability and Accountability Act (HIPAA) for certain covered entity clients (e.g., insurance carriers).

The personal information you provide directly through this website (PolicySlicer.com) for general inquiries or initial insurance shopping is generally not considered Protected Health Information (PHI) under HIPAA, as we do not collect detailed medical histories, specific health conditions, medication information, or Medicare Beneficiary Identifiers (MBIs) through this initial online interaction.

Should you choose to engage further with our services, such as by speaking with a licensed insurance agent as described under "Personal Information You Provide to Us," or if we receive PHI from a covered entity (like an insurance carrier) on your behalf and with your authorization, any such PHI will be handled and protected in accordance with HIPAA and the terms of our Business Associate Agreements.

If your PHI is handled by us as a Business Associate, you have certain rights under HIPAA with respect to that information. These rights are typically exercised through the Covered Entity (e.g., your health plan or provider). Please refer to your Covered Entity's Notice of Privacy Practices for more information on your HIPAA rights.

Security And Account Protection

We have implemented commercially reasonable physical, administrative, technical, and electronic security measures designed to protect against the loss, misuse, and alteration of your personal information, and to meet applicable legal and contractual obligations, including those under GLBA and, where applicable to specific data, HIPAA. This includes, but is not limited to:

  • Secure data storage and transmission technologies

  • Access controls and user authentication

  • Regular security assessments and vulnerability scans

  • Incident response procedures for security breaches

Despite our efforts, however, no security measures are perfect or impenetrable, and we do not promise that your information will remain secure. If you suspect a security breach, please notify us immediately in accordance with the "Contacting Us" section below. In the event of a security breach that affects your personal information, we will provide you with notification as required under applicable law.

We appreciate your help in safeguarding the integrity of your own and others’ privacy. We encourage you to let us know immediately if you suspect that any personal information you shared with us is being used contrary to this Privacy Notice.

Children’s Information

Healthy Labs Inc’s Platform is not directed toward persons under eighteen (18) years of age. We do not knowingly market to or collect any personal information from children under the age of eighteen (18). If you are under eighteen (18), you are not permitted to submit any personal information to us.

If you are a parent or guardian and believe that we have the personal information of your minor child in our database, please contact us immediately at info@PolicySlicer.com to request that we remove such information from our records.

International Transfers

We are located in the United States. As such, the Platform and Services are intended for use in the United States and the personal information that we collect is stored on servers located in the United States. This means that your personal information will be collected, processed, and stored in the United States, which may have data protection laws that are different from (and sometimes less protective than) the laws of your country or region.

By sending us personal information, you agree and consent to the processing of your personal information in the United States, which may not offer an equivalent level of protection to that required in other countries.

Contacting Us

Should you have any questions about this Privacy Notice, contact us by sending an email to info@PolicySlicer.com or writing to us at 8605 Santa Monica Blvd, PMB 38903, West Hollywood, CA 90069. (For CCPA specific inquiries, please use privacy@PolicySlicer.com).

To determine the appropriate retention period for personal information, we consider the amount, nature, and sensitivity of the personal information, the potential risk of harm from unauthorized use or disclosure, the purposes for which we process it, and whether we can achieve those purposes through other means, and applicable legal requirements.

Your Rights and Choices

California law provides Consumers with specific rights regarding their non-GLBA personal information, provided that we are able to verify their identities. This section describes your rights and explains how to exercise those rights.

Access and Data Portability Rights

You have the right to request that Healthy Labs Inc disclose certain information to you about our collection and use of your non-GLBA personal information over the past twelve (12) months. Once we receive your request and verify your identity, we will disclose to you:

  • The categories of non-GLBA personal information we collected about you;

  • Our business or commercial purpose for collecting, selling, or sharing that non-GLBA personal information;

  • The categories of third parties with whom we share that non-GLBA personal information; and

Exercising Your Rights

Only you, or a person that you authorize to act on your behalf, may make a verifiable Consumer request related to your non-GLBA personal information. You may also make a verifiable Consumer request on behalf of your minor child.

An authorized agent is a natural person or a business entity registered with the Secretary of State that a Consumer has authorized to act on his or her behalf. When a Consumer uses an authorized agent to submit a request to know, correct, or delete, Healthy Labs Inc may require that the Consumer provide the authorized agent written permission to do so and verify their own identity directly with Healthy Labs Inc, unless the Consumer has provided the authorized agent with a valid power of attorney. Healthy Labs Inc may deny a request from an agent that does not submit proof that they have been authorized by the Consumer to act on his or her behalf.

To exercise the access, data portability, correction, and deletion rights described above, please submit a verifiable Consumer request to us by:

  • Emailing us at: privacy@PolicySlicer.com

The verifiable Consumer request must:

  • Provide sufficient information that allows us to reasonably verify you are the person about whom we collected personal information or an authorized representative; and

  • Describe your request with sufficient detail that allows us to properly understand, evaluate, and respond to it.

We cannot respond to your request or provide you with non-GLBA personal information if we cannot verify your identity or authority to make the request and confirm the personal information relates to you.

Making a verifiable Consumer request does not require you to create an account with us. We will only use personal information provided in a verifiable consumer request to verify the requestor's identity or authority to make the request.

Response Timing and Format

We will confirm receipt of your request within 10 business days. We will respond to Requests to Know, Requests to Delete, Requests to Correct, and Requests to Opt-Out of Sale/Sharing within 45 calendar days of receipt, unless we need more time (up to 90 days total), in which case we will notify you of the reason and extension period in writing.

We do not charge a fee to process or respond to your verifiable Consumer request unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing your request.
